You will not receive a membership card. Your plan’s effective date will be on file with our office.

**15% discount is for cash and checks.

Program Guidelines

  • Patient’s portion of treatment is due the day of service.

  • No refunds or premiums will be issued at any time participant decides not to utilize plan.

  • Cannot be used in conjunction with financing such as CareCredit.

  • Any discounted services paid by credit card will be reduced to 13% discount.

Patients agree that Ivory Dental fees stated must be paid at the time services are rendered. Any service not paid for at the time of services will be billed at usual and customary fees. Plan fees are only valid when paid at the time of enrollment. This is not an insurance product.

Program Exclusions & Limitations

This program is a discount plan, not a dental insurance plan. It cannot be used:

  • In conjunction with other dental plans, dental insurances, or other promotional offers.

  • For treatment which is the sole opinion of our doctors lies outside their scope.

  • For referrals to specialists.

  • For hospitalization or hospital charges of any kind.

  • For costs of dental care which are covered under automobile medical.

  • For services of injuries covered under workers’ compensation.
